I ................................................................ hereby apply for Membership of the Club Multinacional de Senderismo (hereinafter The Field Club). I understand that The Field Club is a Registered Association whose Members share a mutual interest in its primary activity of walking in the Spanish countryside. I agree to pay the Annual Membership Fee for the current year and accept that should I not pay the Membership Fee for any following year, or be refused renewal by the Committee, my Membership of The Field Club will cease. I also agree to pay such fees relating to my participation in any of The Field Club's activities that may be required.

I confirm that I have received, read and understood the document entitled "Members Walking Guidelines" and that I agree to abide by all the various Conditions and Regulations contained therein and that I will similarly conform to any new Conditions and Regulations that may from time to time come into effect during my Membership. In particular, I confirm that I have disclosed any underlying Medical Condition and any Medication that I am taking that I am medically fit to participate in such of The Field Club's activities as I may book and that I will without delay inform The Field Club should my Medical Condition, or Medication, change in any way. I also agree that I am responsible for ensuring that any Guests I may introduce who participate in the activities of The Field Club are made aware of and agree to conform to the same Conditions and Regulations.

I confirm that I have suitable and sufficient Medical Insurance cover under the Spanish National Health Scheme, or have equivalent Private Medical Insurance cover, to participate in Field Club activities and list the Insurer and Policy number below. In the absence of being unable, or unwilling to provide supporting information I assume all liability for such expenses.

CLUB MULTINACIONAL DE SENDERISMO [CIF G92497080] (THE FIELD CLUB) IS NOT RESPONSIBLE FOR ANY INJURY (OR LOSS OF PROPERTY) TO ANY PERSON INCURRED WHILST PARTICIPATING IN THE CLUB’S ACTIVITIES FOR ANY REASON WHATSOEVER, INCLUDING ORDINARY NEGLIGENCE ON THE PART OF THE CLUB OR ITS OFFICERS, WALK LEADERS, WALK CO-ORDINATORS, FELLOW MEMBERS, OR ANY OTHER PERSONS INVOLVED WHATSOEVER.

In consideration of my voluntary participation in all or any of the activities of the Club Multinacional de Senderismo (The Field Club) I hereby release and covenant not to sue Club Multinacional de Senderismo (The Field Club) or its Officers, Walk Leaders, Walk Co-ordinators, fellow Members, or any other persons involved whatsoever and absolve them entirely and completely from all or any present or future claims resulting from ordinary negligence on the part of the Club Multinacional de Senderismo (The Field Club), its Officers, Walk Leaders, Walk Co-ordinators, fellow Members, and any other persons involved or others listed, for property damage, personal injury or wrongful death, arising as a result of my engaging in the programmes and activities of Club Multinacional de Senderismo (The Field Club), wherever, whenever, or however the same may occur.

I accept that anyone organising a walk or any other activity for The Field Club does so voluntarily and solely for the benefit of the Members and Guests who participate in such activities. I am voluntarily participating in these activities with the knowledge of the dangers involved and hereby agree to accept any and all inherent risks of property damage, personal injury, or death.

I acknowledge that there is a degree of risk involved in such activities and consider myself to be adequately fit and able to participate in and safely complete, the said activities. I have noted all of the guidance provided and in particular as it relates to routes, track conditions, pace, equipment, my health, fitness to participate in and be adequately prepared for, any specific activity. I understand that the level of difficulty relating to any particular activity and these types of activities in general may be affected by prior, prevailing and forecast weather conditions and / or my level of fitness and physical condition.

Further, I am aware that walking and hiking, involve significant cardiovascular stress and physical effort. I understand that all sports involve certain risk, including but not limited to death, serious neck and spinal injuries resulting in complete or partial paralysis, brain damage and serious injury to virtually all bones, joints, muscles and internal organs, and that no equipment is provided by Club Multinacional de Senderismo or its Officers, Walk Leaders, Walk Co-ordinators, fellow Members, or any other persons involved for my protection as may be necessary to prevent serious injury. I further understand that Walking and Hiking involve a particularly high risk of ankle, knee and leg injury. In addition, I understand that participation in Club Multinacional de Senderismo activities involves other non-core activities incidental thereto, including but not limited to, travel to and from the site of the activities, participation at sites that may be remote from readily available medical assistance and possible reckless conduct of other participants.

I confirm that I will read and understand the information contained within the description of each specific activity. If I am in any doubt whatsoever, I will consult with the Walk Leader, or other Officer of the Club as may be appropriate and obtain such information as I deem necessary relative to my safety and well-being during the activity in question.

In consequence I unconditionally accept that neither The Field Club, nor its Officers, Walk Leaders, Walk Co-ordinators, fellow Members, or any other persons involved is at any time responsible for my personal safety or that of my Guests and that my/our participation in any activity of The Field Club whatsoever is voluntary and entirely at my/our own risk. I

therefore discharge Club Multinacional de Senderismo (The Field Club), its Officers, Walk Leaders, Walk Co-ordinators, fellow Members and any other persons involved, from any responsibility or liability for my personal safety and well-being whilst I am taking part in such activities and I hereby confirm and accept that the responsibility for my welfare and safety is mine and mine alone.

I further agree to indemnify and hold harmless the Club Multinacional de Senderismo , its Officers, Walk Leaders, Walk Co- ordinators, fellow Members, or any other persons involved against all claims arising as a result of my engaging in Club Multinacional de Senderismo activities, or any activities incidental thereto, wherever, whenever, or however the same may occur and hereby voluntarily waive all and any claims resulting from ordinary negligence, both present and future, that may be made by me, my family, estate, heirs, or assigns.

I hereby instruct my heirs, Executors, representatives and Trustees, that in case of my incapacity or death they are to uphold and abide by this Commitment and Agreement.

I sign this Disclaimer freely and with the intent that it wilI exonerate the Club Multinacional de Senderismo, its Officers, Walk Leaders, Walk Co-ordinators, fellow Members, or any other persons involved from any liability whatsoever, arising from my injury, or death, whilst participating in any activity organised by The Field Club, even if in strict legal terms there may be such a liability.

I understand that this waiver is intended to be as broad and inclusive as permitted by the Laws of Spain and agree that if any portion is held invalid the remainder of the Disclaimer will continue in full legal force and effect.

Release Waiver

The following signature guarantees Club Multinacional de Senderismo , its Officers, Walk Leaders, Walk Co-ordinators, fellow Members and any other persons involved that, I have read and fully understand this Insurance Disclaimer and agree to the Terms and Conditions stated herein, or have otherwise Declined to sign the Disclaimer by marking the box below accordingly.
